not to be nasty but in High school in psych class, there was this girl who I sat like 2-4 feet away from me, and there was always this really bad odor coming from that direction but I didnt realize that it was her breath. Whenever she yawned or breathed for that matter, I would catch a whiff of it.

The truth is they may be suffering from tooth decay or gum disease. Halitosis is not noticeable to the person who has it. Sometimes a person is allergic to certain foods that cause this also. TOMATOES can really affect some people because of the acid in them reacting with their stomach acid.
Being truthful can be really difficult especially when you are sensitive to the feelings of others. But the truth is better than not communicating. This is a sensitive subject.
Start by saying, "I have something important I need to discuss with you. I think you should see a dentist and I've picked up a few business cards from area dentists, I think you should see one of them. If you need a ride, I'll take you. If there is anything, I can do to help you I will. If the dentist says you don't have a problem then lets try an allergist. I'm telling you this because I care about you and don't want a health issue to interfere with your success."
If you don't have the ability to say it, Cut and paste this into Word and email it or mail it to them.

Worked at a hotel and this guy was a smoker and had bad breath in general. He was trying to tell me something quitey so he leaned in close and whispered. It was so bad. It smelled bad after he left for like five minutes. Oh my god.

Ahhh, my frshmen year advanced algerbra teacher had horrible, horrible, horrible breath. Not just like coffee breath or anything but like chewing on the dogs azz breath. Every now and then you would forget and ask for help with an equation and then be like omg damn it ahhh never mind I got it, I got it, go away.

Bad breath is often caused by dead teeth in the mouth. Maybe the person is afraid to see a dentist. Help them get over their fear. Just tell them, "I think you have a dead tooth. Have you seen a dentist lately?"

I'm a straight forward person so when this actually happened to me I simply smiled and said, "so, how did that dog's @ss taste last night" Needless to say that cured the problem.

Get out some gum/ mints you eat one and then ask them if they would like to have one..
IF they dont.. Just back up a couple of steps..

Carrying gum or mints with you, and offer them one without saying why. If they turn it down and you are friends, I think its appropriate to say something.

I have before. Just offer them gum or mints. What more can you do without hurting their feelings? Hold your nose? :)
