NO you do not require it to leave the country cos you are an american, not a Nigerian.
BTA's are only required for Nigerains leaving the country for a visit abroad.
and you never have to pay anyone for it. its for your own personal use.

From the sounds of it you are being scammed.I hope they havent parted you with your money.
The "person"(I use the term loosely) you are in contact with...is not American.the pictures you have been sent are stolen of a dating website.You do not need to pay some doctor for medicine to save the life of this stranded traveler.
Have a look at internet-love-scams.org and www.fraudwatchers.org
They will give you the complete picture of what is going on.

I don't know that it would be specific to Americans. Most countries have an 'exit tax' to help cover the costs of services such as customs and immigration.
