depends on where you are...
if you are in an english speaking country dont hang out with the people who speaks your language.
if you are not then you should watch a lot of TV. It helps you to understand the language and after a while you will be able to speak it.
reading is not a good way of improving English.
you should also know a lot of phrasal verbs.

First of all, Read English Newspaper daily without missing each and every word. U will definetly find the words which u doesnt know the meaning, u refer that word in dictionary and learn the meaning of it.
Always think in English, I mean to say that normally everyone thinks in their own language. So u try to think everything in English, this helps u a lot in improving ur english.

first thing is people get afraid to talk in english.
dont be afraid of speaking english
take the initiative to talk in english.speak whatever u know and do not bother about the grammar. within some days u will find by urself the mistakes u are finding in your english. u will correct by ur own and since u do it on your own, u wont forget those errors. finally ur english will be good.

Suggestion: listen to radio talk shows, they are usually hosted by very well educated people. I have found that Journalists have high language skills as far as vocabulary is concern. Why radio talk shows? well, because if you just listen, you are focused on listening,
unlike TV shows, you are more focused ont body language and visual aspects.

Better you put your son/daughter in convent school. It becomes compulsary to speak in english. Gradually you'll improve. Its my experience.

simple like i did start listening to good english music with its lyrics in ur hand and then slowly when u start catching the words and joining them , then go for a vacabulory preparation
