I think this question is allowing you to demonstrate your ability for self-easement and self-evaluation, and your general attitude towards problem solving. As you recall a situation you choose to reflect upon, you will need to clearly identify the issue that was at stake; and then a comprehensive understanding of what happened, and what should have happened? Your role in the situation. And how would you act if you are in a similar situation, now!!
You can pick any situation you like either from your previous work experience or from your daily personal routine, but you need to be able to tell exactly how does it relate to the job you are now applying for.

Thats a shitty question....
What you should try to do is twist it so you look good and can show you learnt something positive for the experience.
Something like...
I'm an independent worker and on this project I didn't consult enough with my colleagues now I'm a much better team worker...
I spent time looking at the details and not the overall picture I now know to take a step back and consider all the options. This was such a valuable experinece blah blah bs bs....
If doing this project again i would have assigned more time to the planning stage. I now realise to spend more time in the planning stage to increase my productivity still more to incredible heights that i am now a demi-god.. blah blah.. bs bs

Say something like ordering a strip-o-gram for your mates Stag do and he went off with her, or his fiancee found out - or even that it was his fiancee!....
Do not say - running from the Police Dog handler after mugging an old lady, because it just increased your sentence

I gave someone a second chance when I should have fired them. It was a bad decision because I ended up firing them anyway and giving the second chance just prolonged the inevitable.

i stuck up for a friend who said the she had been harassed by a member of staff,
only to find out a few months later that she did not like him and just wanted to get rid of him.
i felt like such a cow, but was more angry at my friend for what she had done.
