Why would you ask something like this? Michael does have body mismorphic disorder... in which every time he does look at himself... his is dissatisfied. Hence, the multiple facial sugeries. People with such a disorder constantly see themselves as ugly (like anorexic people see themselves fat but are terribly skinny).
