What system are you using? It could be you have a faulty disc. If it worked for you before, buy a cleaner for the system you are using and run that through at least twice. It may be that the disc heads are dirty and can't read the disc.

maybe you modified something in the .dat files
or you incorrectly installed a mod...or
your system does not meet the minumum requirements
check everything....that happened to me a long time ago
I modified that files for the ammo and handling of cars

Maybe you've saved your game at "Maddog's mansion", DON'T
ever save there, because it's curropted & glitched!
Also cheat-codes may have affected the game...

take better care of your software. dont leave your discs laying around. put them in the cases again after you use them. it is likely a scratch. if it isnt a scratch on the disc it could be dirt.
