Yes, give him 2 weeks notice so he can hire someone to replace you.

You won't have any paper work to fill out but your employer will.

You may or may not have to fill out termination papers, That varies with employers.
Politely explain that you are taking another job and thank the Manager/Company for the experience they have given you.
It is always recommended to give two weeks notice to allow the company time to recruit a replacement for you.
Be honest and polite you never know when a future employer will want to contact your past employers for a reference.

It's a two week notice, yes. Some employers may make you fill out some kind of thing, but most do not. It depends on what kind of job you have.

Personally for the first job I held... I had a good relationship HAPPY GO LUCKY with my boss and I told him I was giving him my two weeks notice... I also filled out a paper saying I was giving my two weeks notice, Just to be nice. ... If you have a paper its pretty much so that they (the boss) can file it and state that you requested... well... told them you were leaving. So yeah... just give him/her a paper stating uhh... the date (like today is Aug. 10 ... put Aug. 24th will be my last day. Thanks.) and that gives them your two weeks notice so that it don't look bad on you... :) good luck.
