Dig a whole a bit bigger than the pot. Pop the plant out - trying not to yank by its stem - sometimes it takes some cajoling and fiddling...

Plant in the new hole, back fill with the soil that came out of the hole. Water well.

Do you want to plant it? The generic answer (which might be affected by the exact plant) is to dig a hole twice as big and twice as deep, mix in some topsoil and peat moss, add the potted plant (slice a knife across some of the bound roots before dropping in), firm up the soil and add more soil if necessary. Water nicely and consistently until established.

All that being said, if I know I have a sturdy, hardy plant, I just dig a hole, drop it in, tap it down, and water it.
