Yes i can't recall what machine is used but it involves compressing hydrogen and oxygen at extremely high temperatures.

I don't know what do you mean by scratch. But if you burn hydrogen with oxygen you get water. The combustion takes care of the ratio. If you have more oxygen you will have water and oxygen. If you have more hydrogen you will be left with water and hydrogen
