First who get's to be the judge of what is insane or sane? It's definitely not a position I would want to be in over another. What is sane for one may be insane for another. As for a first sign, I believe it is different for each individual. However if I had to pick something universal I would say the first sign is when you feel that you have lost your trust and belief in yourself.

There are 3 requirements. First, a person has be found to commit a criminal offence. Second, the person has to plead innocence because of insanity (usually now called being of unsound mind). Thirdly, the court has to agree.

Sanity is a legal term, and not very scientific. It just denotes whether the subject can tell right from wrong.
Actual neuroses and psychoses are so absolutely diverse that it's not possible to list all of the possible first signs. Why don't you tell us why you think you're "insane?" We'll tell you if you are.
