1. Do all your homework. Homework, no matter how tedious it may seem, actually helps you learn.
2. Study on a daily basis. If you study on a daily basis, you won't need to cram on tests.
3. Get help if you don't understand the material.
4. Stay focused. Prioritize. Homework, studying first, and then go do whatever you want.
5. Take classes that challege you, but not drive you mad. Perhaps you're taking too many difficult courses at the same time.

Well- You can start by not really worrying about your grades. Center your attention on LEARNING.
Learn everything you can. Read a lot- and from your SN, I believe that you do.
Have you tried getting your parents to help you study? Or perhaps your brothers or sisters, if you have any?
A lot of people have found that playing soft music helps them to concentrate.

Here are my tips from when I was in high school
Sit in the front.
No talking during class.
Review notes and homework before test.
Do homework and even study every night even if there is no exam.
Homework first, it doesnt matter how long you have to hand it in

Take classes you have an interest in if possible. If you like the subject you will usually pay attention more and learn the subject matter easier.

well you need to work hard and to get good grades you need a systematic rational approach to studies. tell me your subjects in HS then i can tell you a good method to get nice grades.
