Pull my underwear out of my ***. I don't care if people see me. My comfort is more important than people's small-minded opinions.

I pick it! I don't give a rats a*s who sees. It's okay. You might be the highlight of someones day. They are going to go home and go right to their roommate and say,
"I was walking down main street today and this chick in front of me picked her butt! It was awesome!"

that never happened to me you have to were the right clothing for your body ,but if it did i would probably bend down with my back facing a wall or something that would help block off public view fix my shoelace and/or just tugg on the bottom of your pants and if its shorts just act like your fixing your clothes like mess with your shirt sleeves then waist area and just pull on the bottom of the shorts

I almost never wear underwear, and when I do it's boxers. So to answer your question, I dunno.
