Basically, it's an incredibly ugly and evil 'uber-octopus' with wings, a primitive God worshipped by human followers, created by a demented British horror writer called H P Lovecraft.
The more common spelling is 'Cthulhu'.
Some people claim they can pronouce 'Cthulhu' accurately, but saying it too often tends to produce a heck of a lot of spit.
The Metallica song is also the title of Lovecraft's first story about this demented seafood dude: 'The Call of Cthulhu', written in 1928.
When Kirk Hammett's having a bad hair day, his whole head looks just like Cthulhu having a good hair day.

Ktulu is another way to spell "Cthulu", a non-euclidian being of such unspeakable horror, just to see him would drive people insane... part of H. P. Lovecraft's Cthulu mythos (with the novel that deals primarily around him, "Call of Cthulu"... other Lovecraftian books that mention him are "Shadow over Innsmouth", "The Dunwich Horror", and "At the Mountains of Madness").
As an interesting side note, "The Thing that Should Not Be" is directly based on Shadow over Innsmouth.

Too much to write here, this will explain. They based it on a H.P. Lovecraft book. Dave Mustaine wrote it, but exited the band before they recorded it. Metallica went ahead and used his writings after he left.
