Once a player has been taken out, they cannot re-enter the same game. If a team is desperate for a long relief pitcher, they can always use another starter that didn't throw in the game, such as in the 18-inning playoff game last year where the Astros pinch-hit Roger Clemens for the end of the game.

There is only one time when a player can be removed and come back later: when a catcher is injured and there is nobody available as a replacement. As for running out of pitchers, the only available option would be to put in a position player. In fact, there was a minor league baseball game about a month ago that lasted over 20 innings. The losing pitcher was an outfielder.

I think you can only use a starting pitcher every other day if one of those appearences was a start, I think that's how it goes.
So say the starting pitcher starts' Monday's game. Then he gets called to relieve on Wensday for someone. I don't think he can come in in relief on Friday and make his next sceduled start on Saturday, the very next day. If he didn't pitch on Friday then I'm pretty sure he could still start on Saturday but otherwise the rules get very confusing.

No, if you've already appeared in a game and then been removed from it, you cannot return. Either you'll need to use a position player (ie Mark Grace or Jose Canseco a few years back) or use another starter (usually one who threw two or three days previous)

No they can't, however another starting pitcher can be used who didn't pitch that day.

i think they can, but usually they put a 3B or outfielder on the mound, because the pitcher can damage their arm
