It maybe that your wax ring has gone bad...(toilet rocking will make it fail, if its not secure).
Or, it maybe that you have a "vent blockage", and when you use the downstairs facilities, such as sink or anything else that uses water, you drain water from your toilet or shower drain, and dont notice it, until the fumes start to come up... (This would be a hazardous condition as it can be explosive, or it can kill you with the fumes)...
I would get it checked out, in any circumstances...

Any chance you have some small leak somewhere or other water (like what drips off the soap and collects in the soap dish)? If you have leaks either under the sink, behind the toilet, etc. the water can collect in even small amounts and smell. If leaks get onto any wood then that will smell. If you had kids other places for potential odor are on the back of the toilet right where the seat hooks on and at the foot of the toilet in the front.
There really shouldn't be any "bathroom odor". If there are no leaks and any rugs or covers are clean; and if you are using a clean mop for the floor, there's no reason for there to be an odor (unless its immediately after use of the toilet).
I have found Comet cleanser very effective for cleaning and leaving the bathroom smelling good.

Try using Lysol liquid in the toilet. You may have to get on your hands and knees and scrub the base boards with it as well. Have you thought about maybe it could be the water? If you have well water I know it leaves an odor at times. If the city is cleaing out the lines it can also leave an odor. You may have to pour lysol in the toilet every time you use it for a while.

Vinegar doesn't kill germs, try bleach. 1 part bleach to 10 parts water, spray it on a surface, let it sit for 10 minutes then wipe it up. Make sure you don't spray any soft surfaces that could be ruined by bleach.
Also there is always the possibility that it isn't bathroom odor. If a mouse (or other animal) had gotten into your walls and died in the vicinity of your bathroom, it could make it smell pretty awful. I don't know if theres anything you can do about that.

They have fresheners that you can put on timers to go off every few minutes.Have you tried Fabreze?It seemed to do the job.I heard that Oust is a good air freshener.I had a rug in my bathroom that i just had to throw out.I have three kids and one of them is 3.He use to miss the toilet.I threw the rug out.I should've wiped it down and sprayed the Fabreze,just started using it and it's great.I don't smoke but have friends that do.I didn't think I could get the odor off my couch but it worked.I never heard of using vinegar but I think Fabreze or Oust will do the trick.It didn't mask it for a while it got rid of the odors for me.

Try Vinegar and Baking soda in the sink, toilet and tub drain.
