It is not an orgasm because he is acting. If you've never seen them live in any way shape or form, whether it was on TV or for real, you wouldn't know. But Gerard really gets into it when he's on stage. He acts and jumps around and all that jazz, as do the other band members, but mostly him since he's the one with the mic. HE'S ONLY ACTING!
Okay, in detail: There is no sure fire way to tell, because they never show him, and he never says "Hey look, I'm havin' an orgasm!" That's about as good as I can put it.
