All the time - I always say I'm going to get out to my grandparents on a particular day, and it always happens on that particular day I'm beat down from work, or not feeling well, and I end up apologizing endlessly.. but hey I always make it up to them, I'm out there the next day ... something like that.

without ill intentions, if I meant to do something and then it turned out badly, I would apologize.
I would not knowingly do something, know the outcome and then apologize. That would be two-faced.
