No. You can just say thanks in person. A TY note seems way too formal. They are only paying you what they think you are worth.

A formal thank-you for a significant raise isn't customary in business, because it is the company (or in your case the doctors) is thanking ~you~ by giving you the raise. Compensation is also a private issue, so you don't want to bring attention to the fact that you got a big raise when other people in the office may not have. You can thank them in private if you've got good rapport with them - if not, just smile and do your job with an extra bit of happiness (and wealth!).

In writing? Absolutely not. If the raise is because of your performance with the job, then a respectful, but short and quick, verbal acknowledgement is all that is required from you.

If it was a major raise, wouldn't be out of line, but not necessary either. Would be a nice gesture though. For a normal annual raise, no.

I don't know about a thank you note, but maybe you could take food(?)
or baked goods and tell them you just wanted to show your appreciation
