Some water, some shade, meal worms (from a pet store), flies (remove a wing so they have to hope around). That should do it.
Best let it go under some bushes, in a damp place.
If you keep him/her make a little cave in its cage, old bark, moss.
Don't worry about too much food. However, crickets and meal worms do smell after a while.

Why on earth would you keep a toad? anyway, keep it's environment moist and swampy, they like shallow water with some leave. They eat bugs, so go ahead and catch some flies or something. Catch them alive.

Go to the exotic pets section of About.com, and do a search for toads.
About.com has really excellent references for petcare.
