Moss with some bark beneath it is a good substrate. The moss absorbs moisture well. Be sure to spray the habitat twice a day to ensure the humidity is adequate. I would go with an aquarium, so the substrate doesn't dry so quickly. Also, have a pool of water available. Most like to swim or soak.

They like crickets and meal worms, which I would purchase from the pet store. Crickets outdoors have harder exoskeletons.

Make sure to get the moss and bark from a pet store. Also, I would get a water conditioner for the water you use. Toads can be sensitive to chlorine just like fish.

This depends on what type of toad. Some are more aquatic than others (such as firebelly toads) and needa tank that is half land, half water. Others can live in an environment that is damp with high humidity, but very little sitting water.

If it is a very aquatic one, I would recommend getting at least a 10 gallon tank (with a secure lid) and lots of aquarium gravel/stones. Form the stones to make a steep slope, so one half can stay fairly dry while the other is submerged. I would also get a Whisper filter. These creat a small waterfall effect while also cleaning the water and put this in the submerged section. Make sure to also put in some things like aquarium plants and larger rocks for it to rest on/in and water conditioner to remove the chemicals.

For a less aquatic toad I would still get a 10 gallon with a secure lid. Instead of water and stones though, the bottom should be covered with dirt and moss- you can buy these at most pet stores for reptiles and amphibians. Also include a shallow water bowl for sitting and drinking as well as fake plants for hiding.

Both environments must be kept fairly warm and humid. You can probably feed the toads a combination of crickets and worms that you can get at pet stores (other ones could contain harmfull pesticides or other things from outside). Some toads get fairly large in size and generally toads and frogs will try to eat whatever they can fit in their mouths.

Consult a book, website, and/or pet store employee for more detailed information, as the type of toad will change how it needs to be kept.

They should be housed in a tropical terrarium with an under tank heating pad at one end and a thick layer of soft, slightly moist soil. Hiding places such as bark or roots should be provided. The terrarium must be well ventilated and the substrate changed frequently.
