Now...speaking from experience.. No nonsence answer with no creams and no fuss is to get them banded... Now the bad bit... This means going in for day surgery... they give you some drowsy juice and then lay you on your side and they open up your **** with a speculum (a surgical version of a hand vice) then they slip elastic bands over the piles and you go home.
