first of all, don't be scared. i'm not going to say that it is the most pleasant thing ever and that i hang out at the gyno office, but i can say that you should not be afraid. your gyno should know that it is your first time, and should explain everything to you.
you'll get asked medical history, which takes a while, so make sure you are familiar with any related disorders in your family, especially your biological mother's health. you'll get weighed and measured (height), and they'll take your BP. you'll have to undress and wear a paper gown, just be warned that your butt will be cold.
the next part is the least fun, but doesn't take long: yes, you will have to lie there on a bench with your privates exposed to a doctor with metal objects. i'm sure that you'll be talked through this by the doctor as it's going on. if your doctor uses the "spotlight", that will be the most awkward feeling. i know i wasn't used to having lamps beaming down there. you're probably most afraid of the speculum, which "holds you open". it's uncomfortable, very uncomfortable, but it probably won't hurt you. if anything, it's just cold.
and the doctor will do an internal exam, where fingers are inserted to make sure that all the parts are in the right place. that takes about 30 seconds.
overall, it's not bad if your doctor makes it comfortable for you. you'll be fine. :)
ps- make sure you're showered and clean, and don't have sex within 24 hours or longer of the appointement.

Make sure you take a shower and wash really well. Basically, you will undress and cover up with a paper "sheet." You will put your feet in these things called "stirrups" and lay on your back. The doctor will shine a bright light in between your legs and put an instrument called a "speculum" into your vagina to hold it open so he/she can look inside and examine you.
It is a little uncomfortable and kind of embarrassing, but it isn't the worst thing in the world. I believe your mother is required to be in the room with you since you are a minor and you can always ask the doctor to warm up the speculum so it isn't as uncomfortable.

First, you fill out some forms about your period and sex life. Then, you will be asked to undress.
You will be given a paper gown or sheet to cover up. Then the doctor will com in ask you some questions about your period and your infection, and will probobly go ove the forms with you. Then, he will examine your breasts and make sure there are no problems. After that, you will lay down on a table with your feet in sturrups. You will feel like your gonna fall off.
Then, he will warm an speculum with water and put in to your vagina. It hold the vagina open.
He will look inside and make sure everything is OK. Then, you will get a Pap smear. The doc sicks a really large cotton swab and swipes some cells of your cervic. Then, he will put gloves on and put some lube on it. he will insert his finger(s) into your vagina and push on your tummy. Then, he will Give you meds for your infection. You don't really need to prepare yourself. RELAX RELAX RELAX!!! It will only be uncomfy if you don't relax
