If it's just a dent... take off the inside door panel and pop it out with a 2X4 and hammer... make sure you hit it in the right spot. this will take out most of the dent... unless it's crinkled.

depends on size, scratches gouges...imposible to answer here, take in to body shop. They all give free estimates.
