No, unless you had sex with aliens who implanted their seed directly into you with their implantation beam.
You had maximum protection possible. The only thing more effective is not having sex at all.

The only way that there is NO chance of getting pregnant is to abstain from sex and sexual activities that put his penis anywhere close to your vagina. That is the only 100% positive way to keep from getting pregnant.
If he wore a condom and used it properly, and if you took your pills exactly as directed, then you should have little reason to worry.

There is no such thing as a fool proof birth control method. So, yes it is possible. How do you know he didn't ejaculate? Sperm are microscopic, and they are present through the entire act of having sex. They don't all restrict release to final orgasm. If he is wearing a latex condom that is of good quality and new, and she is on birth control pills. Then it is not likely percentage wise that she will be impregnated. But, it is possible.

Yes you can still get pregnant ,but if you are taking your birth control the right way, you wont get pregnant, wearing a condom doesnt protect you from diseases either so be careful .

Its very slim. The pill is 92-99.7% effective on preventing pregnancy plus a condom was used so I would say to stop stressing about it.
