I was just wondering if anyone here wanted to help me make my form work. If you do just email me jethaz@gmail.com and i`ll send you what I have for the form so far. I just need help with the finishing touches making it work. I will be very greatful to thoose who choose to help and fix it!

the real problem is that when ever the form is submitted it comes up blank... i have just a few lines wrong, and well honestly i dont know how to make it right!

You need to elaborate a little bit on what you are trying to do and what your problem is, before anyone will offer to help. I see several posts a week from people trying to put together forms that will be emailed to them when their webhost does not support any type of scripting.

Is the problem with the form or with the back end?
