Lol its an old(ish) phrase I belive used to describe the practice of not wearing underwear. Stems back to the days of WW2 when British "commandos" felt that wearing underwear restricted movement and caused you to sweat a lot, thus causing rashes and other problems.
So many decided to do away with the undergarments and wear just trousers.

I have no idea. Maybe commandos don't have time to change or wash their underwear.

maybe the term is more polite way of saying not wearing any underwear. more of a fancy word lol, who knows.

It means that you're not wearing underwear.

Commando is like all impressive; so some people thing it's brave and impressive (but definately not me lol)
