Try just putting in the new OS disk in the drive, ( make sure that your booting from the disc drive ), Let it take over and it should write over and delete you old OS

Try deleting the partition when the option comes up, then re-creating it. That will get rid of the old operating system, along with everything else.

If the OS is on a different partition, you have to format that one.

Your hard disk has bad sectors in it, it is of no use it cant be repaired, you simply have to scrap it, that is the reson it is not being formated, even fdisk is not able to format it.
