I hope you mean a "deserted" island. It wouldn't matter on a desert island because you'd probably die in a few days anyway.
I'd take a lighter. Fire is important for warmth, which you need more than food and water to survive. Smoke is important as a rescue signal, too.
You could find another way to clean your teeth. You don't need soap to survive. A knife would be a close second for me, just because a sharp blade is handy for so many things necessary to survival.

The knife. Branches can be cut to use as toothbrushes, some roots can be crushed to s a soapy pulp for cleaning and fires can be lit in manners other than a lighter. Plus as the first person mentioned it'd last longer. Toothbrushes wear out, soap runs out and so do lighters. At least with the knife you have half a chance of resharpening it!

The knife. It is the key to making all kinds of other things. You could start with a home-made stone knife, but a real one is so much easier to use!
You can make a toothbrush from a stick with the end smashed.
You can make fire with a fire-bow, but you need a knife to make that.
You can make soap by boiling animal fat in a solution of wood ashes, but you need to first make a hunting bow or spears using the knife. Then you will have food AND soap!

knife. I could use it to kill food and cut other vegetation to eat. I could build a fire by rubbing sticks together (yes it would take a long time but is possible). I could then make lye soap from the ashes and fat from any animals that I kill so I would then have soap. I could also use the knife to cut wood, vines, rushes etc. to make lodging and even items of comfort as sleeping mat and chairs. I could also make a toothbrush from wood and moss, may not taste great but would keep my teeth clean.

a knife i can rub two sticks together to make fire so i dont need a lighter i can carve a toothbrush and find something to use for the brissels and the soap well im sure i could find something that comes pretty damn close

A Lighter- so I could create a big smoke signal like on Pirates of Carribbean (minus the rum). Hey, it'd easier than rubbing sticks together to make fire.

3. Lighter.
Make fire. Find sharp stick. Spear fish with it. Cook fish over fire. Eat fish. Of course, if I liked sushi, I would go with the knife and whittle to pass the time. But since I think "sushi" is Japanese for "ewww, they're eatin' it," I'd have to go with the lighter.

The knife, you can build and kill with it. I can make a fire without a lighter. Water is everywhere so soaps's not that big of a deal.
