It is always at the Johnson Space Center in Houston. There may be a backup plan to use another site if a tornado or something takes out Johnson during a flight, but I have not heard about it if there is one.

No, the Marshall Center manages the Chandra program as well as other ones like the Hubble.
As soon as the shuttle clears the tower, it always has been Huston.
