There's a big rivalry here in Australia in the A.F.L between the West Coast Eagles and the Fremantle Dockers. The two teams bases are all of 30km apart in the one town. The Dockers represent the working class, while the Eagles represent the scardonay sipping, upperclass. Each game between the two is called a "derby". And there was an infamous derby a few years ago which was named the "demolition derby" because of several all in fights (which resulted in a total of 15 match suspensions and tens of thousands of dollars in fines).

Assuming when you say all of sports you mean all of sports and not just in the US, here are a couple and the reasons
1 Australia v England in Cricket for the Ashes trophy
Why? Its history. The first test match was played between the 2 countries in 1877 and the rivalry has been intense ever since. The game of cricket was invented by the English and it was a matter of pride that they could beat the colonials at the game. It reminded the colonials who was really the boss. Nowadays the "Colonials" take great pleasure in beating the English at every opportunity.
2.Liverpool FC V Everton FC in the English Premier League
They have been playing against each other since 1892
Everton was formed in 1878, then in 1892 there was a split in the club and Liverpool FC came into being. The Derby is played with all the passion that history can create.
3 The Ryder Cup in golf. First played in 1926. It was originally played between the US and Britain until 1973. Then it became The US V Britain and Ireland. Since 1979 its been played between The US and Europe. The competition is played every 2 years and alternates venues between the 2 competitors. What makes the Ryder cup stand out is the history and the fact that this is a team event in a sport that is mostly played in an individul result format.
These are just 3 rivalries in world sport of note. You could also throw in Australia V New Zealand in Rugby for the Bledisloe Cup,
Argentina V Uraguay in Sth American football, Australia V the US in Swimming & Barcelona V Real Madrid in Spanish Football, just to name a few

Sox and Yanks. Personally I think that the Sox are overated! The yanks aren't to bad either. I don't mind either team, but Johnny Damon was my favorite player on the Sox, but then he went to Yanks, so now I kinda like the Yanks, because Damon kicks butt!

Pittsburgh Steelers vs. Cleveland Browns, no matter how bad the Brown's have been lately this is still the top rivalry in the NFL. Duke and Carolina are college, yes they have a big following but college rivalries aren't quite the same and basketball is nowhere as near as popular nationwide as the NFL, plus who cares about the south.

I WOULD SAY either Havard VS Yale- oldest rivalry
Texas-Oklahoma Wild West shootout-beset colledge football rivalry
Cubs Cardinals-consistant rivalry in baseball, even though for many seasons the cubs have finished in last place but may have taken a couple of series from the cardinals.

I would say Michigan vs. Ohio State at the Big House in Ann Harbor. There's so much history with both schools. So many Championships and Heismans, can't deny that this is the best ever.

Without a doubt...India v.s. Pakistan in cricket. These countries hate each other, and have raged war for many years over the Kashmir region. An India/Pakistan cricket match is literally life and death for many people. Duke/North Carolina and Ohio State/Michigan are awesome, but can't touch India/Pakistan in terms of sheer hate and detest for the opponent.
