Depending on the job for which you are applying, only list what will be seen as an asset to them. By no means lie, but try to see your activities from the employer's perspective.
For example:
If you are applying to an animal shelter, do NOT list hunting as an activity.
If applying for a retail job, do NOT list gambling as an activity.

This can be both hobbies and volunteer or philanthropic activities. This is the section that shows that you are well rounded and balanced. If possible (without lying) try to list an athletic activity (which shows an interest in your health, and that you are not lazy), a volunteer activity (shows that you're not just in it for the money) and an educational pursuit (like an art class, to show an interest in your continuing education.)
However, don't worry too much about this section...it is rarely a deal breaker. Good luck to you.

It means "For the Attention of - Job Application" in other words the post room at the company you are writing to will direct it to the department dealing with the job application and then when they receive it they will know it is a job application. In some companies all job applications are opened at the same time and looked at at the same time so they want to be able to identify a job application from ordinary post.

It can mean volunteer work, unless there is a separate section for that as well. Write about your volunteer work, and extracurricular activities if you are still in school. You need to impresss them by revealing how vaired of a person you are in a section like that. Good luck!

All you've mentioned, and any organizations of which you may be a member, charities, etc..
In many organizations, such as sales, those are considered to be "prospects" for business. Also to explore the broader aspects of your interests.
