4 teams qualify.

1. Wolverhampton Wanderers, in the 1950s, when the great Billy Wright (first player to earn 100 England Caps), Wolves were the team to beat. Before the advent of the European Cup, they were considered Europe's best after they beat Honved Budapest (most of the Mighty Magyars were in the side)

2. Aston Villa. They were dominant in the early part of the League, winning Cups and titles, but it took a 71 year wait before they triumphed again in 1981, when a strike force led by Peter Withe, flanked by Gordon Cowans, amongst others, won the title. Villa would go on to win the European Cup in 1982, with Withe scoring the winner.

3. Derby County.Owed much of their success to the team of Brian Clough and Peter Taylor. Leading them first to the old 2nd division title in 1969, before topping the great Liverpool and Leeds sides of the early seventies, by winning the title in 1972. Clough and Taylor fell out with the board, but his replacement, Dave Mackay (a famous player both with Derby and Tottenham), led them to another title in 1975.

4. Amazingly, Clough and Taylor went back to the 2nd division and resuscitated the fortunes of Nottingham Forest, one England's oldest clubs, guiding them to promotion in 1977. Forest then went to dominate the 1978 season, winning the title from a very strong Liverpool side, who reigning European and League Champions. Forest would then emulate Liverpool's succes in Europe, dethroning them in a 1979 first round tie, enroute to winning the trophy in Munich.
Forest again were crowned European Champions in 1980, overcoming a Kevin Keegan inspired Hamburg 1-0 in Madrid's Santiago Bernabeu. Along the way, Clough and Taylor had a falling out, but Forest did win another 2 League Cups in 1989 and 1990. However, despite leading them to the 1991 FA Cup Final, Clough would be denied the one trophy missing from his domestic cabinet.

Overall, I think Villa have around 12 or more trophies won ahead of Forest's 9. They have won more League titles but Forest has won 1 more European Cup than them.

But considering how long Villa have stayed in the top flight and Forest's position in League 1, with Derby and Wolves in the Championship, the logical choice at this time would be, Aston Villa, as the midlands most successful side.

Nottingham Forest or Aston Villa

The Forest team of the late 70's were far superior to any Villa side, but Forest have gone downhill big time lately so Villa might just shade it.
