Just go to your closest Social security office with your drivers license and birth certificate. They may have upped the ID anti lately since I last had this happen to me 4 years ago..but you can eventually get one from your local SS office with the amount of trouble directly proportional to the amout of ID's you have.

Go to your local social security office. Every county must have one. Take along two forms of ID, your birth certificate and also two things (like letters in the mail) showing your address. They should be able to help you out from there.
