In the UK, we do verbal warnings, followed by three written warnings.

However, I'm not sure "insubordination" / walking out of a meeting is necessarily a good reason for a warning in the first place.

What if your employee is superstressed too? You could have a lawsuit on your hands for inhumane practices in the workplace. Your employee's emotional health and safety must be taken care of too....if he / she can come up with a list of instances where they have felt bullied or belittled then you could be in trouble.

Sounds like you need to improve communication with this person, and operate on a less personal basis, going from the tone in your question.

Most places I know, insubordination is immediate grounds for dismissal. Either way, it's not something you should handle in a memo.

A) Why did they walk out.
B) Talk to he/she in person and ask them why.
and C) If you are their boss I would let them know the importance of responsiblity and if they dont change their additude or take your suggestion seriously and ACT, not just talk about the changes they will make then I would polietly let them go.

id leave a meeting you were giving too if i was them....i dont know how you would ever move up in a company and earn any respect when you have to go on yahoo answers to get help with employees......lol......you are a weenie d*ouche bag push over

suggest anger management classes!!! then blame it on his parents. This actually happened to a friend of ours. Blamed it on how his dad didn't play ball with him as a kid??? or maybe he can run the meeting next time and see if he/she likes or tolerates rude behavior.
