When does it have to made and if someone gets injured who can replace him?

To be eligible to be on the playoff roster a player must be listed on the teams 40-man roster by Aug. 31. If a player is injured during a playoff series he cannot be replaced until the next round begins.

Playoff rosters must be set at 25, not including disabled players, on August 31. For each player on the 60-day DL, teams may add players to the eligible list during the playoffs at the same position, provided that they were in the organization as of August 31. This is why some teams will bring up injured minor-league players and put them on the 60-day DL. Teams must choose 25 players from their playoff eligible list before each round of the playoffs.

Rosters have to be turned in for the playoffs, before they start. Once a round starts, an injured player cannot be replaced. Once the round (Division, League Championship) is over, you can replace a player for injury, however, that injured player cannot return to the active roster the rest of the playoffs.
