An injured player may go to the minors and play there. It might be a little more complicated than that, but that's what I understand.

Additional details: Pitchers may remain in the Minors on rehab for no more than 30 days and position players for no more than 20 days. All players with more than 3 years of service must consent to a rehab assignment (the team can not force him to go). Players can also be shifted between levels of the minor league system. They can start in AA then go to low-class A then to AAA, etc.

It depends on some factors, but if a ML player is put on Rehab, he is still listed on the DL. He is placed on one of the minor league teams (usually will start close to Spring Training site). For example, with the Yankees...a player will start his rehab at High A (Tampa), as that is where the Yankees Player Development & Spring Training site is. That way, high officials can keep tabs on the player at the start of rehab.
If all goes well, after a few games the player will move up a level (depending on the extent of the rehab). After a few minor league starts, the player will come off the DL and go back to his team.
Most depend on what the injury being rehabbed is, to determine where the player goes, and how long they stay there.

From the MLB collective bargaining agreement, Article XIX, Sec. C:
"...a Player on the Disable List may be assigned to a Minor League club for up to a maximum of 20 days (30 days for pitchers) for each injury, or reoccurance of an injury, for the purpose of rehabilitation."
There's also wording in there stating that the player has to give his consent for such an assignment.
