He can change at anytime between pitches but not after he gets 2 strikes on him. Once he has 2 strikes he must stay at whatever side he was at.

Sure can, though a pitcher has to declare which arm he'll be using to pitch if he can throw both ways. And yes there is at least one player who can do this right now. Can't think of his name though, I think he's still in college or just graduated/was drafted.

Yes. You can change sides of the plate between pitches.

only if there is a pitching change made. once a batter is in the batter's box to face a pitcher, he has to stay with the side he's on.

yes he can, I saw it happen in a MLB game last year. Can't remember who the batter was though.
