My wife and I run into this problem quite a bit....we make more money than most of our friends do, so we seem to get the extra on the bill a lot.
Once in awhile I don't mind picking up the appetizer or the dessert or something but it seemed to be getting to be a habit, so I found that it is easiest to grab the check right away when it comes to the table.
Figure out what you owe and lay the cash down. Don't forget tax and tip. My friends were notorious for that to. I would leave a tip to cover the whole bill, because the server shouldn't get screwed because my friends are cheap.
If it is just the two of you out, and you lay your money down first, all she will do is subtract the amount you put down from the total and pay the rest. Most of the time if your friend trusts you and you have been in the habit of picking up that little extra, she won't even add her costs up.
If she does and says something to you, just say you have paid more than your share many times and you have paid for what was yours now. She owes the rest.
Either that or pay for your drinks as you get them. Don't run a tab. You can also tell her to put a dollar amount in the middle of table, and you do the same, then just use the money to pay for drinks as you go, split whatever is left or use it for tip.

Go somewhere together. A place where you really like or can do something fun and ask, hmmm...how much money do you have? Well, we can't do that b/c I only have $1 guess we will just have to go home. Try this a few times. If you never really do anything then maybe she'll say I have money to do......whatever and then she pays.
Or you can be bold and ask her what she has money to do today. If she has no money then say you can't go b/c you have no money either. Or say we could do this if we had xxx amount of money. Ask how much she can contribute.

Before you go out just say that its her turn to pay. If she doesn't want to then don't go out with her. A real friend would be only too pleased to share the cost.

First of all, she is NOT your best friend or she would not be using you that way. Secondly, tell her you won't be going out any more because it is simply too expensive for you. Invite her over to watch TV. If you really want to go out, ask another friend to go with you and make sure they know up front that it is "Dutch Treat."

just get there and pretend you forgot ur wallet.. do it a few times, she'll get the hint
and if u both r really that close, just saying in a very friendly way, "hey, sorry i cant treat u today i'm broke" should be somethin she might feel a little bad about.. but will soon appreciate...
