My cousin is currently living with my family and he is 19 years old, I hear groaning and screaming in the bathroom and no one else is in there with him. I only live with one guy which is my father and he uses the bathroom upstairs, so I don't hear him. But I never heard of guys being in pain when in the restroom or is it a thing guys experience? I don't want to ask him because he'll ask me why was I listening, but if I hear a sound I just hear it.

Don't be afraid to ask. Obviously he is amking enough noise that you were able to hear him. So just say that. Sometime when he is in there moaning knock on the door and ask, "Are you ok?"
Then see if he admits anything.

Maybe he unfortunately "caught" his, "thing" in his zipper....that would be cause for screaming for sure.
