A kneaded eraser should work on graphite, try moving to a softer lead, 4B up to 9B. White nylon erasers will work, but as another said, if you've used enough pressure to dent the paper, you're out of luck. There's a soft black eraser on the market that works great on both as well.

Could be your kneaded eraser is too old n dirty, it should be really soft and shouldn't break into pieces, and still light grey. Also if it's picked up too much oil from your skin it won't work well anymore.

What density of graphite are you working with? You can get this white erase at most art shop that works pretty well, but if the graphite has creased the paper there is no erasing that. sometime just a regular no.2 pencil eraser works well. I also keep sand paper in my pencil box to shape my erasers.

Believe me ... bread (freshly baked english bread or sandwich bread, soft with moisture) can be real good eraser.. I use it to erase charcoal and graphite. Try it!
