Perhaps English is your second language. I point that out because proper grammar usually coincides with proper manners. How do you cut meat with one hand by the way? Manners vary across the country. Relatives from back east, visiting me on the west coast had a problem with picking up fried chicken with their hands. You just don't see that many people in California cutting their chicken with a knife and fork at KFC.

It does not matter to me what hand is used, as long as people keep their elbows off the table. If they realized how much room they were taking up at the table maybe they would stop? Now that isn't a proper way to eat!

There are those that think that the same as you do. But that custom was developed for desert cleanliness conditions. Since we have better water here, customs are different.

Moreover, the left hand should be reserved for removing dirt, as when cleaning oneself after going to the toilet (istinja’) and blowing one’s nose and other kinds of cleaning oneself and removing dirt. How can a person use that which is used for removing dirt and impurities for putting food in his mouth?

I believe that people act accordingly to the way they were raised, as well as the people they acquaint themselves with. If your parents raised you to eat with your napkin in your lap, one hand crossed across your lap and only one hand on the table (unless while cutting meat, or eating fried chicken etc.), no hats, sit up straight, and keep your mouth shut while chewing, then kudos to them and you. Not everyone has manners!

I think I understand your question. Using a knife and fork simultaneously while shoving food into your mouth is vulgar. Once you cut meat or whatever is on your plate, put your knife down before putting food into mouth.
