Those are thought bubbles so that the Sim can speak to you in something other than Simlish. :)

If the bubble has a red icon inside, like the shape of a toilet, that sim needs to get to one BAD! :) They usually wave their arms when they are upset and have those kind of bubbles, when they are tired, hungry, irritable cos they haven't had enough fun recently, and other such things.

Other bubbles can be rather random, like maybe they are dreaming, thinking or talking about another sim or some topic like politics, love, and other things.

Some times a sim who doesn't like to talk about fashion or weather will get annoyed when someone has the bubble with those topics and won't change the subject. Depending on which sim it is, you can click on the sim to either change the subject or keep talking about that subject, depending on if you want your sim to be friends with the other one. You can find out the topics your sim likes best by checking the head icon on the bottom right for their personality info. If you want your sim to get better at some topics, they should go downtown and buy some magazines to read in the topics your sim needs to brush up on. Sims that love all kinds of topics tend to make friends faster and don't cover their ears and stomp off when someone else is boring them. lol
