It's an infarction in his right thigh muscle.

He had a muscle infarction. His explanation in one of the first episodes:

A patient: "What made you a cripple?"
House: "I had an infarction."
The patient: "A heart attack?"
House: "It's what happens when the blood flow is obstructed. If it's in the heart it's a heart attack. If it's in the lungs it's a pulmonary embolism. If it's in the brain it's a stroke. I had it in my thigh muscles."
The patient: "Wasn't there something they could do?"
House: "There was plenty they could do, if they made the right diagnosis, but the only symptom was pain. Not many people get to experience muscle death."
The patient: "Did you think you were dying?"
House: "I hoped I was dying." [#101]

As others have said, he has a muscle infarction. His girlfriend at the time of the accident (Stacy) didn't want House to have his leg amputated, so he has been suffering for years with chronic pain (hence him taking pain killers for relief a lot) and can only move around with the use of a cane.

Yes, he has a thermometer stuck up his butt.

It happened because of an incompetent doctor he visited when he was a child. It is still in his butt and cannot be removed because if it breaks, the mercury will be released and poison him. He was then inspired to become a doctor when he grew up to bring quality medical care to everyone so they would no be crippled by an incompetent doctor like he was.

He had a clot on his leg.. If it would have travel to his head would have been a Stroke.. He could have lost his leg.. It is that serious.. Good Luck!!

he had a blood clot in his leg and it caused major muscle death leaving him with a limp and constant pain.
