Caulk maybe? That's what we're supposed to use to seal cracks in our concrete houses here, and I've used it to seal gaps between my concrete walls and patio.

I had something called hydraulic cement applied to stop leaking, but it was on the walls. I don't know if it would work in the area you have a problem.

the patio needed to have some pitch to it away from the house, only thing i would try is dry loc paint
