That depends on what is required for the paper. Also, we need more information. It's a self-assessment, but in what context? Something for 6th grade would be a lot different than something for the workplace, etc.

Ouch! Self assessment is one of the hardest things to do!
Why are you writing this? It's obviously an assignment, because nobody does this for fun, but why was it assigned? Because that could have a bearing on what you are doing.
If you're looking at it to help you make career decisions, look at your past performances outside of the classroom. That is, how are your leadership skills? How supportive of other people are you. How good are you at coaching? Can you work well with other people, blend in to make an effective team player, or are you best at being a super star and standing out from the pack?
Moving inside the classroom, how do you handle assignments? Are they last-minute efforts, or do you give them due consideration and put a little extra effort into them. Or are you right in the middle?
Are you achieving your potential as far as marks go? Could you do better, or are you pretty much at your peak?
Sorry, can't really do much for you because there isn't really much information given in your question. But maybe this can be a starting point.

I. introduction--general information regarding self assessment, purposes, pros and cons, etc.

thesis-- statement 3 reasons about urself that u are going to be assessing

II. Body

a. 1st reason (from thesis)
b. 2nd reason (from thesis)
c. 3rd reason (from thesis!)

III. conclusion


this is pretty much a good outline for ANY paper u have to write. u give a general introduction regarding the topic, then state what u are going to write about, and u go in depth in the body. then u conclude by discussing what u've learned, or the results of ur analysis, and u close with a strong statement that summerizes it all. good luck!
