The people that get all built and huge are doing two things...exercises that promote muscle growth (weight training) and eating properly - including lots of protein which promotes muscle growth. Someone who is working out but not getting larger is probably doing lots of cardio-vascular work (like running, which can be catabolic), or is not eating properly.

There are a lot of factors that are involved when it comes to muscle gain. First off is body type, some people will show muscle growth and much faster gains while others will not. Next is the training aspect of weight lifting. Things such as eating right, training correctly, and getting enough sleep. Also the actual routine a person is using can also play a part.
