The process is the same as a regular ultrasound. The difference is that they will scan "targeted" areas which just means that the scan is more detailed. The ultrasound machine used will be a better machine.
The examiner will look at the fetal brain, heart, stomach, kidneys, spine and umbilical cord site. He/she will determine the position of the fetus, the amount of amniotic fluid present and the location of the placenta. So, as you can see, it is a much more detailed scan.

A Level II ob ultrasound is usually ordered because of an abnormal blood test, certain health problems, or increased risk of a chromosomal abnormality.
A Level II exam is perfomed on the same type of equipment as any other OB ultrasound. They look the same, feel the same, and take only slightly longer to perform. It entails obtaining additional views for the doctor to evaluate.

I work at an outpatient imaging facility. Every OB ultrasound we do, after about 16 weeks, is a Level 2. I start with the maternal cervix, placenta and anionic fluid levels. Then we turn our attention to the baby. We start by measuring the brain (in two different areas), abdomen and femur. This gives us an estimated due date and an estimated fetal weight. We document the posterior fossa and ventricle of the brain. We image both lenses of the eyes and the nasal bone. We document the fetal spine, from base of skull to tailbone in two different planes. We document the 4 chambered heart (and heart rate), stomach, 2 kidneys, diaphragm, aorta and arch, fetal cord insertion and bladder. We document a 3 vessel umbilical cord, with Doppler wavelengths. We then document both forearms/hands and lower extremities/feet. We also check the sex of the baby (but only if the parents want this information). At last, I document the mother's gallbladder, as pregnant women have a higher risk of gallstones. The sonographer will give you a guided tour of your baby, and it is quite an awesome experience.
