I have Photoshop 6. With the picture on the screen, there's a button marked T on the left. Click that, and draw out a text box where you want it. Click inside the text box and type.

If you've got your image open in Photoshop already, change to the Text Tool (big "T" in the tools), position your text by clicking in the photo (you can change the positioning later -- it doesn't have to be perfect placement right now) and start typing. To move the text, cursor over the photo until you see the cursor change to a 4 direction arrow and then you can click and reposition the text. If you've already commited your text, you can change to the Move Tool with the same 4 direction arrow and move the text layer or any other layer you want (the layer has to be selected in your Layers menu). I have a photoshop on my other computer, I believe there is a "A" icon on your right. That is your text button. Click on it, type what you wish. Voila. As for saving go to File, then save as. Type in what you want the name to be and then save.
