My son just had that recently. The doc told me to make sure that i give him lots of water and Pedialyte. make sure he stays hydrated. Depending on how much the child is going will depend on how much pedialyte you push.
If it persists, i would call the pediatrician

It depends on how long your baby has had diarrhea, color of it and how thin it is. If you notice blood or your baby has a temp highter than 100, you definitely should call your pediatrician right away.
Hopefully your baby is just having transitional or typical thin stools. Sometimes what seems like to us too thin of stools is not what a pediatrician would call diarrhea. Good luck to you.

As with the other answers...keep the child well hydrated. No sugary drinks as the sugar could make the diarrhea worse. Take the baby to the doctor's. If you have introduced anything new into his/her diet, stop feeding it to him/her. If you have changed formula, go back to what you were using before. I would suggest going to the doctor's though.

My baby had the runs for 11 days straight at this age. I knew she wasn't dehydrated since she drank nonstop. I called the doctor three times during this time. My pediatrician said that she was probably teething and to just keep feeding her. She also advised against baby wipes at this time and just wetting soft paper towels, like Viva so you don't irritate your baby's bottom anymore than it already is. Good Luck!

Keep the baby hydrated. That's the first step.
What color is the diarrhea? That is a HUGE sign.
Call the doctor, just to be sure. Chances are s/he'll tell you just what they told us--is he running a fever? Okay, don't worry.

when my daughter had diarhea (she was older than your son though, 10 mos) i was told to not give her formula for 24 hours, only pedialyte, as much pedialyte as she would drink. and yogurt with live/active cultures in it. yogurt is a very good remedy for stomach problems. although im not sure if a doctor would recommend yogurt to a 4 1/2 month old, i would call your doctor to see what advice they have for you. diarhea can be very serious in babies, if your baby is showing signs of dehydration (ex: no tears when crying, no wet diapers for 5 hours, or sunken eyes) you need to take him to the ER.

keep giving your baby fluids.
call your pediatrician. every pediatrician has a different procedure on what to do when your baby is sick. find out what they suggest and when they want you to give up treatment at home and come in for a visit.
but if you are concerned...never take chances with your baby's life.
