First I would get it professionally cleaned. If the smell still exist sprinkle baking soda on the carpets and let stay for couple days, and vacuum up. It that doesn't get get, place some white vinegar in a small open container (sit it where it won't spill) and that should do it.

I put a box of dryer sheets under my seat, Gain smells the best, my friend puts hers right on the floorboard so when her air or heater is on the air blows on the sheets. She smokes in her car and if it helps that it will surely help old odor

Spray febreeze all over the interior and carpeting so that it's a little wet, leave all the doors open to let dry and it should be fine. If that doesn't work, take it to a detailing place and have it shampood

I don't know if it will work on that old odor or not, but I know on odors such as fish and rotten meats charcoal works pretty good.

you need to get some air freshner that gets to the molecules i orderd some before because i was sick in my car it worked good....
one other way would probably be to get it steam cleaned.
